Improving Hospital Discharge

Have your say on hospital discharge – join the conversation

With winter fast approaching, anything that we can do to improve hospital discharges so that they are timely and safe is critical. The Registered Managers Locality Forum report set the benchmark for better discharge practices. Since then, changes have been introduced to try and address the concerns raised. Now we need to know if these changes are making a difference.

Join our provider webinar on 11th December to hear from the Home First Hub and Virtual Ward team, share your current experiences to help identify what is still not right and work with practitioners to help shape next steps. This is also an opportunity to learn about the support that is available from the Home First Hub and Virtual Ward.
