Improving Hospital Discharge Webinar

Norfolk County Council are inviting providers who work with Norfolk & Norwich Hospital to join an upcoming webinar focused on improving hospital discharge processes.

Register to attend here.

Why Attend?

  • Connect directly with hospital teams, the Home First Hub, and the Virtual Ward and hear about the support that they can offer.
  • Help tackle discharge challenges, identify quick wins, and agree on actions that build confidence and trust.

What’s Driving This?

The Registered Managers Locality Forum report (Skills for Care, 2024) set the benchmark for better discharge processes. Since then, changes like Care Act assessments before discharge have been introduced.

Feedback needed: Are these changes working for you?

Your Voice Matters

  • Complete a 4-minute survey: hospital discharge survey
  • Share what’s working, what’s not, and where improvements are needed.
  • Influence practical next steps through local webinars with hospital and Home First Hub staff. (Survey covers N&N, James Paget, and Queen Elizabeth hospitals—informing future East & West webinars.)

Why Now?

With winter approaching, timely and safe discharges are critical. Your input will help improve care, reduce delays, and strengthen collaboration.

Complete the survey today and join NCC on the 11th of December!
