These tools are more than paperwork. When used properly, they can transform the quality of the Annual Health Check – making it more personalised, more meaningful, and more effective.

The Pre-health Check Questionnaire: Why is it important?
Why the Pre Health Check Questionnaire Matters
The Pre Health Check Questionnaire helps individuals, families, and care staff think ahead about what is important to raise at the appointment.
It gives the person time to reflect on any new health concerns, changes in behaviour or wellbeing, medication questions, long-term conditions and, screening, vaccinations, or follow-ups.
When completed in advance, the pre-health check questionnaire helps to reduce anxiety, support communication, ensures important concerns are not forgotten, and helps health professionals prepare for a more focused and productive conversation.
The films demonstrate how completing this in partnership leads to better outcomes.
What is a Health Action Plan?
The Role of the Health Action Plan
The Health Action Plan is what turns the conversation into action.
It clearly sets out:
- What was discussed
- What needs to happen next
- Who is responsible
- Timescales for follow-up
A strong Health Action Plan supports continuity of care, accountability, clear communication between professionals, providers, families and the individual, and better long-term health management.
Most importantly, it ensures the person understands what will happen next.
